Dear Referring Doctor and Staff:
For your convenience, we are providing this list of dental insurance and discount plans for which we are contracted as Preferred Providers.
We are available Monday through Friday from 9:00 to 5:00 at any one of our three locations; we also offer early morning and evening appointments monthly. Our goal is to see your patients as soon as possible. If you do not see your patient's insurance plan listed, please call our office with any questions. We offer finance options and payment plans for treatment.
